Family Case Management Program

The family case management program serves all pregnant women, infants and children who meet eligibility requirements. This program assists new mothers and families by providing information and assistance with immunizations, nutrition, doctor visits, and other needs vital to family health and safety. All services are free of charge. Family Case Managers can help pregnant women and other families who meet income guidelines apply for medical benefits through the various programs offered by the Department of Public Aid.
